Please complete the design team evaluation form. Fill out the top portion of the form (course, project title, team name, and team members) and forward it to your liaison engineers for completion. The form should only be completed by sponsor personnel that have regularly interacted with the team and are in a position to evaluate the team's efforts.

Forward the completed evaluations to Dr. Stanfill no later than 12 December.
